Junior Player Profile & Parental Consent Form

By submitting the Junior Player Profile Form online, a parent/carer will be acknowledging and agreeing with the following:

• Children and Young People: Safeguarding Policy and Procedures
• Photography Consent
• Code of Conduct for Parents/Carers of Young Golfers
• Kent Golf's Social Contract (replaces the Code of Conduct for Young Golfers)

In addition, in submitting the form each parent/carer is advised that, in compliance with current data protection legislation, all efforts will be made to ensure that information is accurate, kept up to date and secure and that it is used only in connection with the purpose and activities of Kent Golf. Information will not be kept for more than one month after the period to which it relates . The information will be disclosed only to those officers and volunteers of Kent Golf for whom it is appropriate and relevant officers of England Golf where necessary.

It is the responsibility of the junior and their parent / guardian to notify the County Secretary if any of the details change at any time. 

The Junior Player Profile and Code of Conduct for Young People are valid from 1 September to 31 August and have to be renewed annually.
